Below is the link for the working papers.  A parent/guardian will need to complete the form below and send it back to me with a copy of the birth certificate.  I will mail you the blue working permit that will need to be signed by the student.  The student always keeps the original and the employer makes a copy of it. If you want to pick it up at the high school office please let me know and I will email you back when it is ready.   You can email the completed application and a copy of the birth certificate please Email Monica Johnson.
